We're looking for a proactive, results-oriented "hunter" who enjoys developing new business, strengthening existing referral relationships, and identifying admission opportunities across key referral markets. The ideal candidate is energized by being in the field, comfortable calling on physicians and healthcare partners, and motivated to grow hospital census through exceptional relationship management and clinical expertise.

Position Summary

The Clinical Navigator reports to the Director of Business Development and serves as a key liaison between referral sources, patients, families, and the hospital. This role is responsible for cultivating referral relationships, managing an assigned territory, conducting patient assessments, and supporting the admission process. The Clinical Navigator collaborates closely with the business development team, Admissions Manager, admission staff, nursing, and other interdisciplinary team members to facilitate timely and appropriate patient admissions while driving census growth.

Key Responsibilities

  • Proactively develop new referral relationships and expand existing partnerships within assigned markets.

  • Serve as a visible presence throughout key referral territories by routinely calling on physicians, hospitals, case managers, discharge planners, skilled nursing facilities, and other referral partners.

  • Identify and pursue new admission opportunities to support hospital census growth.

  • Build and maintain strong relationships with physicians, case managers, discharge planners, ICU and floor nurses, payer representatives, patients, and family members.

  • Conduct clinical assessments to determine patient appropriateness for admission.

  • Analyze referral and admission trends to identify opportunities for market growth and improved outcomes.

  • Partner with admissions and clinical teams to ensure a seamless referral and admission experience.

  • Educate referral sources on PAM Health's rehabilitation services, clinical outcomes, and patient success stories.

  • Provide recommendations to improve customer service, referral conversion, patient satisfaction, and quality initiatives.

  • Represent PAM Health with professionalism, integrity, and a patient-centered approach.

Qualifications

Qualifications

Education & Licensure

  • Degree in a health-related field from an accredited college or university preferred.

  • Current, unrestricted licensure in one of the following is required:

    • Registered Nurse (RN)

    • Respiratory Therapist (RT)

    • Physical Therapist (PT)

    • Occupational Therapist (OT)

    • Speech-Language Pathologist (SLP)

Experience

  • Minimum of two years of clinical experience in the licensed discipline required.

  • Rehabilitation hospital or LTACH experience preferred.

  • Previous physician liaison, clinical liaison, healthcare marketing, or business development experience is strongly preferred.

  • Demonstrated ability to build relationships, develop new business, and effectively manage a referral territory.

  • Self-motivated, highly organized, and comfortable working independently in the field.

  • Excellent communication, presentation, and interpersonal skills.
