Overview

 
*Preps and scans inpatient and/or outpatient paper portions of the EHR medical record
 
* Accurately analyzes and quality checks inpatient medical records for completeness along with abstracting medical provider data
 
*Scans and/or files loose documents into the EHR
 
*Enters any deficiencies into EHR in a timely manner
 
*Notifies physician(s) of deficiencies and removes deficiencies within the EHR when completed.
 
*Assists physician(s) in completion of their medical records to include assisting with EHR training, password reset, remote support, etc.
 
*Proactively works to minimize the number of medical record delinquencies. Takes initiative to creatively work with the healthcare providers to complete their records.
 
*EHR skills including but not limited to: prepping, scanning, analysis, indexing, destruction and quality control.
 
*Works to ensure compliance with HIPAA requirements.
 
*Retrieve discharged paper portions of the EHR from the nursing unit(s)
 
*Assists HIM Director in maintaining EHR destruction schedule.
 
*Tracks and trends H&P, Procedure, and Discharge Summary documentation compliance along with medical record delinquency compliance and any other statistical information requested by Director.
 
*Quality check outpatient medical records to ensure they have been prepped and scanned real-time during the patient’s treatment. Assist HIM Director with quality audits. Also, ensure outpatient medical records are stored and destroyed per HIM Policy.
 
*Responsibility for answering and routing incoming calls, and takes complete messages.
 
*Maintains the highest level of customer service via courtesy, compassion, and positive communication.
 
*Safeguards the confidentiality of patient records from unauthorized review or release.
 
*Works in a manner that promotes safety; wears clothing appropriate to the performance of the job.

Responsibilities

The HIM Coordinator is responsible for the overall management of medical records, coding, and release of information and health information processes. The HIM Coordinator is consistent with policies and procedures as set forth by accrediting and regulatory agencies.

Qualifications

Education and Training: Prefers the HIM Coordinator to be a graduate of a Health Information Management program with subsequent passing of the registry examination (RHIA) or accreditation (RHIT), but not required.

Experience: 1-3 years (1-3) experience in a Health Information Management Department is preferred.
